BROWSE THE GUIDE TO SERVICES FOR THE UNINSURED (8.9 MB)

The Colorado Consumer Health Initiative would like to invite you to use our guidebook of services for the underinsured and uninsured: Connecting Care and Health in Colorado, for your referral needs. We created this guide to help community leaders like you who are serving the uninsured health consumers in Colorado find the resources and safety-net health clinics for your clients.

A training session is also available for the guidebooks and can be provided free of charge for you and your co-workers to better understand the depth of the healthcare crisis and how to use the guide to help your clients. This is a guide to help consumers:

  • Understand the complex maze of health and human services programs in Colorado;
  • Identify for which programs and services consumers might be eligible; and
  • Locate safety net programs and resources throughout the state.

SECTION ONE

Includes eligibility and other program tables, and program summaries. Section One may be used for:

  • Understanding the changing healthcare environment and its effects on the safety net;
  • Quick identification of program eligibility and populations served;
  • A summary of programs throughout the guide; and
  • Determining where to find more information on programs.

SECTION TWO

Includes information on health and other programs, providers and other resources. Section Two may be used to identify more in-depth information on:

  • Available medical and public assistance programs and public health programs;
  • Health, dental, mental, and long-term care providers; and
  • Other resources and programs for assistance with food, energy, legal, housing, interpretation, environmental, transportation, and other related issues.

SECTION THREE

Includes a listing of safety net agencies, departments, and providers. Section Three may be used to locate agencies, departments and safety net providers and clinics in their local communities where consumers can obtain assistance with eligibility determination or receive services.
